Add your questions, the uploads you need and your consent forms, and save the set as a request pack. A section can show only to the people it applies to, based on an earlier answer.

Send the link, or let patients register
Add the patient and their private link goes out by email. Or put a registration link on your website: they enter their name and email, and their own link is emailed to them. No account, no password.

WorkZerk reminds, you review
A reminder goes out at 7 days and again at 14 if required items are missing, and you can change both day counts. You approve each upload, or reject it with a reason and they are asked for a better copy.

Sensitive answers are masked
Mark a text, date or upload request sensitive. The answer is stored encrypted, shows masked to your team, and only admins can view it in full. Sensitive files open only for admins.
Every view is logged
Each time someone views a sensitive answer or opens a sensitive file, WorkZerk records who, when and from which IP address. The log is a report you can filter, print or export.
Consent recorded by version
Keep each consent form and your privacy policy in the terms library. An acceptance records the exact version, the date and time, and a typed name or drawn signature if you ask for one.
Sign your existing PDF
Already have the consent form as a PDF? Upload it and each patient signs it on their phone. You both get a signed copy with a signature record page at the end.
Two-factor sign-in before uploads
You cannot add an upload request until two-factor sign-in is on for your login, because uploaded documents sit behind it. Every upload is checked for viruses.
Deleted when you choose
Sensitive information is kept until you delete it. You can instead set it to be deleted 30 days, 90 days, 1 year, 5 years or 7 years after a portal closes.

Is WorkZerk a practice management system?
No. WorkZerk collects new patient forms, signed consents and documents, and keeps them on a record for each person. It has no appointments, no clinical notes and no billing, and it is not a medical record. It sits in front of the system you already use: read what came in, or export it to CSV.
Does a patient need to create an account or install an app?
No. Each patient gets a private link to their own portal. They open it on a phone or computer, fill in what you asked for, and can come back later to finish.
Who in the practice can see what a patient entered?
Anything you mark sensitive is masked for everyone and can be viewed in full only by admins. Each person you invite to the account is an Admin or a Member, and for a Member sensitive answers stay masked and sensitive files will not open. Text, date and upload requests can be marked sensitive. Choice questions and signatures cannot.
Is it certified for health privacy laws?
We do not claim certification under any health privacy law, and you should check what applies to your practice. What WorkZerk does: everything is encrypted in transit and at rest, answers marked sensitive get field-level encryption with a separately held key, only admins can view them, every view is logged, and two-factor sign-in is required before you collect documents.
Can new patients register themselves from our website?
Yes. Generate a self-registration link and put it behind a button on your website, or turn it into a QR code with any QR tool. The patient enters their name and email and is emailed their own private link straight away.
How much does it cost?
One plan at $9.95 a month with every feature. 25 people can be onboarding at once, with no limit on how many you put through in a year.
