Every waiver signed and every allergy known, before the day.
Share one registration link or QR code. Each attendee fills in their details, medical and dietary information and emergency contact, then signs the waiver, photo consent and code of conduct on their phone. On the day, check-in shows who has arrived.
30-day free trial. No credit card.
Sound familiar?
Waivers get signed on the bonnet of a car
Forty paper forms on a clipboard at the start line, three of them wet, and one parent who has already driven off.
The allergy is in row 63 of a spreadsheet
The survey tool gave you a spreadsheet. The caterer wants a list, the first aider wants a different one, and medical details are sitting in a file anyone on the team can open.
You do not know who has not sent it back
Thirty forms are in. Nobody is sure whether the camp has thirty-four children or thirty-six, or whose consent is missing.
How attendee registration works in WorkZerk
- Step 1
Build the form once
Add your questions, the waiver, photo consent and code of conduct to sign, and a section of extra questions that only shows to the people it applies to.
- Step 2
Share one link or QR code
Turn on the self-registration link and put it in your confirmation email, on your website or on a QR code. Each attendee types their name and email and is emailed a private link to their own form.
- Step 3
See who is finished, and who has arrived
A reminder goes to anyone who has not finished. Export the answers to CSV for the caterer. Turn on check-in and attendees tap Check in when they arrive.
What you can collect from an attendee
- Contact details and date of birth
- An emergency contact
- Medical conditions and medication, marked sensitive
- Dietary needs and allergies
- A signed waiver or release
- Photo and video consent
- Code of conduct accepted, by version
- The name of the child a parent is signing for
- A license, certificate or medical clearance, as an upload
Waivers accepted by version
Each waiver is a versioned terms document. An acceptance records the exact version shown, the date and time, and a typed name or drawn signature.
Medical details stay masked
Mark a medical question sensitive. The answer is stored encrypted, shown masked to your team, viewable in full only by admins, and every view is logged.
Extra questions only where they apply
Ask whether they are under 18, staying overnight or bringing their own equipment. A section of follow-up questions shows only on that answer.
A count of what is missing
Each form shows as finished, in progress or not started. The Requests screen lists who still owes you something and which item is holding the most people up.
Answers out as a CSV
The Request answers report puts every answer in one table and exports it to CSV, for the caterer, the first aider or the coach.
Check-in on the day
Attendees tap Check in on the link they already hold. You get a live count and a roster of who is on site and who has not checked in. You can check people in yourself.
$9.95 a month, every feature included.
25 people can be onboarding at once, with no limit on how many you put through in a year. Colleagues who work in the account with you are $4 a month each.

Is this a ticketing system?
No. WorkZerk does not sell tickets, take payments, allocate seats, manage sessions or print name badges. It also has no cap on how many people can register and no waitlist. Sell the ticket or take the booking wherever you do now, then send the people who booked your WorkZerk link to collect their details, waivers and consents.
How does an attendee register?
They open your link or scan your QR code and type their first name, surname and email address. WorkZerk emails them a private link to their own form. There is no account or password, and they can stop part way and come back. WorkZerk gives you the link: you make the QR code from it with any QR tool.
Can a parent sign for a child?
Yes. The parent registers with their own name and email, a question asks for the child's name, and the parent types their own name and signs. One email address gets one form on a registration link, so if a parent may have two children attending, ask for all the children's names in the one form.
Who can see medical information?
Mark the question sensitive and the answer is masked for your team and for the attendee on their own link. Only the account owner and colleagues set as admins can view it in full, and each view is recorded with who and when. Text, date and upload questions can be sensitive. A multiple choice question cannot, so ask for medical details as a text answer.
We have hundreds of attendees. How does the 25 people limit work?
The plan includes 25 active people at a time. An attendee is active while they hold a live link on a portal you have not closed. Going over 25 never blocks a registration: each extra active person costs 5 cents a day, about $1.50 a month, paid from credit you add to the account. So 200 attendees open for 14 days is 175 extra people at 5 cents for 14 days, which is $122.50 on top of the $9.95. Close the portal when the event is over and the count drops back, with every answer and signature kept. Closing also switches off their links, so do it after the day if you use check-in. One more limit: the account can send 1,000 emails a month, and each registration sends one.
How much does it cost?
One plan at $9.95 a month with every feature. 25 people can be active at once, with no limit on how many go through in a year. Colleagues who work in the account are $4 a month each. Try it free for 30 days, no credit card.
