New member forms filled in on their phone, not on a clipboard.
A new member scans the code at the front desk and gets their own link. They enter their details, answer the health questionnaire, sign the waiver and accept the house rules. If your club has to hold a copy of each member's license, WorkZerk tracks when it expires.

Sound familiar?
The clipboard at the front desk
A walk-in fills in a paper form while a queue builds behind them. Later somebody types it up, guesses at the handwriting, and the email address bounces.
Where is the signed waiver?
It is in a filing cabinet, probably. Nobody can say which version of the house rules that member agreed to, or on what date.
The license ran out in March
The club holds a photocopy of every member's license, and the expiry dates live in a spreadsheet one committee member updates when they remember.
How member onboarding works in WorkZerk
- Step 1
Build the new member form once
Add your questions, the health questionnaire, the waiver, the house rules and any document you need a copy of. Save it as a request pack and reuse it for every intake.
- Step 2
Put the link on a QR code
Turn on the self-registration link and print it as a QR code at the front desk, or put it on your website. A new member enters their name and email, and their own private link is emailed to them. No account, no password.
- Step 3
WorkZerk chases, you approve
Anyone who stops halfway gets a reminder. You approve or reject each uploaded document, and a rejection emails the member to send a better copy.
What you can collect from a new member
- Contact details and date of birth
- Emergency contact
- Pre-exercise health questionnaire
- Liability waiver, signed on their phone
- House rules or club rules, accepted by version
- Firearms, competition or boat license, with its expiry date
- Dive certification card or pilot medical certificate
- Proof of age or student ID
- Your own custom fields, saved to their record
A waiver signed on a phone
The waiver is a terms document the member reads in their link. Ask for a typed name, a signature drawn with a finger, or both. WorkZerk records the date, the time and the exact wording they were shown.
House rules accepted by version
Every edit to your rules is saved as a new version. Each acceptance stays pinned to the version that member read, and the terms history report lists who accepted what, and when.
License expiry dates, watched
Require an expiry date on a firearms license, a competition license, a certification card or a medical. WorkZerk marks each one current, expiring soon or expired, and can email the member 30 days before it lapses.
Who holds a current, approved copy
The document compliance report lists your members down the side and your document types across the top. Only copies you have approved count, and members who are not compliant are listed first. Export it to CSV for the committee.
Questions only for the people they apply to
Ask whether a member has an injury or a medical condition, and show the follow-up questions only to those who say yes. Ask which discipline they shoot or race, and show only that license request.
Ask again every year
Some documents print no expiry date, such as a medical clearance letter. The time to ask again automation requests a fresh copy a set number of days after you approved the last one, 365 unless you change it.

Does a new member need to create an account or install an app?
No. They enter their name and email on the registration page, and WorkZerk emails them a private link to their own portal. They open it on a phone or computer, fill in what you asked for, and can come back later to finish.
How does the QR code at the front desk work?
Turn on the self-registration link for your new member portal and you get one web address. Make a QR code from it with any QR tool (WorkZerk does not make the code itself) and print it. A walk-in scans it, enters their first name, surname and email, and receives their own link by email. If they lose the link, registering again with the same email re-sends it.
Our club has to keep a copy of every member's license. Can WorkZerk track when each one expires?
Yes. Make the license an upload that requires an expiry date. The member uploads a photo or scan and enters the date, and you approve or reject it. WorkZerk then shows it as current, expiring soon or expired, and can email the member before and after the date. The same works for a firearms license at a shooting club, a competition license at a motorsport club, a certification card at a dive club or a medical at a flying club. WorkZerk does not check that a license is genuine. It keeps the copy the member uploaded, the date they entered and your approval.
Is it gym or club management software?
No. WorkZerk does not take membership fees or direct debits, book classes or facilities, or control door access. It collects and keeps the paperwork: forms, signed waivers, accepted rules and dated documents. Your membership system stays where it is, and you can export what you collected to CSV.
We have hundreds of members. How does the limit of 25 work?
The 25 is not your total membership. A member counts while they hold a live link on a portal you have not closed. Contacts with no live link are free, so every member can stay on file. If you sign members up in groups and close the portal when each group is done, you never pay more than the base price. Closing a portal keeps every document and expiry date, but it stops the reminder emails to those members and takes them off the document compliance report. If you keep every member on an open portal so their license reminders keep going out, each member above 25 costs 5 cents a day, about $1.50 a month. A club keeping 200 members live would pay about $262 a month on top of the base price. If that is you, talk to us about a fixed price.
How much does it cost?
One plan at $9.95 a month with every feature. 25 people can be onboarding at once, with no limit on how many you put through in a year. Colleagues who work in the account with you are $4 a month each. Try it free for 30 days, no credit card.
